- Birth certificate: mandatory for a child born on or after 01-10-2023 under current UIDAI guidance.
- Mother/father/legal guardian should accompany the child and provide required details/authentication.
- Additional Proof of Identity, Address, Relationship or Date of Birth documents may be required depending on the case.
- Bring originals for verification.
Do not upload Aadhaar numbers, OTPs, biometric data or original documents to this website. This request form intentionally does not ask for a full Aadhaar number.
